New Patients
To register contact our Reception Staff who will provide registration forms for each member of your family. These must be signed by the patient or guardian. You will be offered a 15 minute new patient medical appointment with one of our Practice Nurses. Please remember to bring your completed registration forms to this appointment along with some form of of ID which includes your name and address. We also ask you to bring any regular medications with you to this appointment.
If you live outside our practice area please phone Primary Care Support England on 03330 142884 who will be able to advise you with regards to your nearest GP Practice.
Change of Details
Please notify us of changes in name, address or telephone number as soon as possible.
Temporary Patients
If you or your relatives are visiting the area for no more than three months you can register with us on a temporary basis – ask at Reception.
Overseas Visitors
Many countries outside the European Union have no arrangements with the UK for free health care. You can still be seen by the Doctors but you may be charged for your consultation, treatment, prescription or hospital visit. It is wise to have adequate health insurance cover.
Registering with the Practice
You are most welcome to register with us as long as you live within the practice inner area. Patients who move address can remain with the practice so long as they remain in the outer area.
